Factors Associated with Depression in Intentional Self-harm In-patients

Abstract
Purpose: According to the statistics of WHO in 2017, the suicide rate of South Korea was ranked 4th in the world. It is urgent for South Korea to establish mental health policy for depression and early detection, in order for prevention and management of suicide attempts. The purpose of this study is to investigate status of depression among the patients, who are hospitalized with intentional self-harm, furthermore to utilize the current situation as basic data, establishing mental health promotion policy of intentional self-harm patients.

Methods: Data was selected from the KNHDIIS(Korean National Hospital Discharge In-depth Injury Survey) database which was produced by KCDC(Korea Center for Disease Control and Prevention) from 2014 to 2015. Frequency, chi-square and logistic regression analysis were performed on patients with total intentional self-harm and with whom classified in presence or absence of depression. The statistical program, STATA 12.0, was used, with significance level of all analyzes set as 5 percent.

Result: Among the total 1,218 patients, 339 patients(27.8%) had depression. As a results of analyzing depression factors: risk factors [financial problems(OR, 4.446, 95% CI. 2.813-7.028), psychiatric problems(OR, 3.984, 95% CI. 2.806-5.656), physical problems(OR, 2.721, 95% CI. 1.571-4.711) and family conflicts(OR, 1.830, 95% CI. 1.217-2.753) than unknown(OR, 1)], gender[female(OR, 1.594, 95% CI. 1.199-2.120) than male(OR, 1)] were significantly higher. While longer length of stay(OR, 1.013, 95% CI. 1.005-1.022), residences [city (OR, 0.571, 95% CI. 0.425-0.767) and country(OR, 0.615, 95% CI. 0.402-0.941) than metropolis(OR, 1), death [yes(OR, 0.406, 95% CI. 0.187-0.882) than no(OR, 1)] were significantly lower.

Conclusion: In order to prevent suicide due to depression, systematic collection is necessary, of information on hospitalized patients after intentional self-harm. Based on the research, the suicide rate can be reduced by establishing the medical support system: high?risk group's advance grasp, continuous treatment, integrated medical service support system, and linkage plan of community-centered.
